Memorial Tree Program Application Form

Plaques may be purchased from any vendor you select. Two available vendors in Janesville are Gramke Monuments and Milton Lawns Cemetery. Please remember the standard size of 2" x 10" for the plaque.

Please return the following information with your payment of $300.00 per tree with checks made out to the Community Foundation of Southern Wisconsin (CFSW) - Shade Tree Fund.

The Janesville Shade Tree Advisory Committee is pleased to help you in the selection and planting of a memorial tree in a location of your choice. A representative of the committee will be in touch with you to determine exact sites and even meet you at a specified location prior to planting. We will install the bronze plaque in a permanent block for mounting next to the tree.
